On-Call Coverage: Hospice coverage required when on call
The Home Care RN provides skilled nursing care to patients in their homes, including hospice coverage when on call . The RN functions as a case manager , completes required OASIS documentation, coordinates interdisciplinary care, and ensures compliance with State, Federal, and Vermont Board of Nursing regulations .
Pay PackageAll-Inclusive Rate: $77.00/hr
Calculation (St. Albans, VT Standard CONUS)Estimated Weekly (lodging + M&IE): $1,246/week
Hourly GSA Cap (37 hrs): $33.68/hr
Non-Taxable Stipend: $33.68/hr
Taxable Base Rate: $43.32/hr
Weekly Gross: $2,849.00

Taxable: $1,602.84/week
Complete and maintain accurate OASIS documentation per agency, State, and Federal requirements
Assess patient and caregiver needs using the nursing process
Develop, communicate, and implement individualized plans of care
Serve as Case Manager , managing caseloads and visit productivity
Provide skilled nursing care including IV therapy, wound VAC, complex wound care, and infection control
Coordinate interdisciplinary services (PT, SW, HCA, hospice, etc.)
Supervise LPNs and HCAs per scope of practice and agency policy
Educate patients and caregivers on medications, treatments, and discharge planning
Maintain accurate documentation in McKesson , including visit lock-down and follow-ups
Coordinate safe discharges and transfers
Participate in QA/PI initiatives, peer review audits, and case conferences
Advocate proactively for patient and family needs
Active Vermont RN license
Home Health experience strongly preferred
OASIS experience required
Hospice experience required for on-call coverage
Strong case management and documentation skills
Valid driver's license and reliable transportation
