Licensed Practical Nurse (LPN) - CARS Outpatient Clinic

Licensed Practical Nurse

Licensed Practical Nurses at Cayuga Health System are an integral part of the collaborative interdisciplinary team devoted to the delivery of comprehensive and compassionate care to the Tompkins County community. LPNs are able to practice at their full scope of practice, and have the opportunity to engage in the development, implementation, monitoring and evaluation of programs and services with the goal of improving patient outcomes, and execute an optimal plan of care in a shared governance environment.

Required Skills and Experience:

  • License: Active New York Nursing license, out of state nurses are still eligible to apply
  • Minimum education: completion of an accredited LPN program
  • Certifications: BLS (new hires have the opportunity to obtain these certifications during introductory period)

Location and Travel Requirements:

  • Onsite Ithaca Outpatient
  • Less than 25% Travel

Physical:

  • Repetitive use of hands and fingers (e.g., Use of computer keyboard); may require lifting and carrying light loads (up to 40 lbs), including boxes, equipment, IV solutions and stooping and kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves and to file documents in lower file drawers); sitting, walking, or standing for long periods of time may be necessary; must be able physically to operate the equipment used for the job.

Pay Range Disclosure:

  • $24.04 to $27 per hour

Position Summary:

The LPN provides direct clinical support in the outpatient substance use treatment setting, focusing on medication administration, withdrawal symptom monitoring, proper and timely documentation, and performing and assisting in basic medical procedures. The LPN works under the supervision of the RN or provider and contributes to a respectful, stigma-free environment for recovery.

Key Responsibilities:

  • Administer oral and injectable medications per provider orders and NYS scope of practice.
  • Perform point-of-care testing (urine toxicology, glucose, pregnancy tests, etc.).
  • Collect vitals and document signs and symptoms of withdrawal.
  • Monitor and report changes in patient condition to RN or provider.
  • Prepare medication doses and maintain controlled substance inventory logs per regulatory requirements.
  • Assist with patient education and support harm reduction strategies.
  • Participate in clinic safety procedures, emergency drills, and incident documentation.
  • Maintain professional documentation in the EMR.

Qualifications:

  • Current NYS Licensed Practical Nurse (LPN) license.
  • CPR certification required.
  • Experience in addiction medicine, behavioral health, or community health preferred.
  • Strong communication and teamwork skills.
  • Ability to work sensitively with individuals experiencing stigma, trauma, or active recovery challenges.
