Are you certified as a Family Nurse Practitioner and looking for a flexible work option? ACT Medical Group, Inc. is looking to hire a visiting Nurse Practitioner. ACT Medical Group is a California and Nevada-licensed healthcare company that provides quality medical care to our home-bound patients.


Overview:
We are seeking FNPs per diem to visit patients homes, apartments, or nursing facilities, in order to both establish new patients, as well as provide follow-up care. This is a per diem position. Our team will schedule you with Medicare patients (referred from home health agencies) per your availability, in your local area. Nurse Practitioners will be able to access patient charts, input patient notes and diagnosis codes into our EHR system (Office Ally), as well as order labs and/or prescriptions for patients, and narcotics (provided practitioner obtains a DEA license), via the online pharmacy. This will be a collaborative agreement between the practitioner and our supervising Physician. Nurse Practitioners will act as PCPs for ACT Medical Group.

  • CLARK COUNTY AREA (NEVADA): Las Vegas, North Las Vegas, Henderson, Boulder City, Mesquite, Laughlin, Paradise, Spring Valley, Sunrise Manor, Moapa Valley, Moapa, Mount Charleston, Searchlight, Enterprise, Goodsprings, Jean, Winchester, Whitney, Sandy Valley, Bunkerville, Primm, Indian Springs, Blue Diamond, Summerlin South, Nelson, Roach, Sloan, Glendale.

Additionally, we cater to other regions based on specific needs and requests.


Requirements and Qualifications:

  • Master's Degree.
  • Board certification and license (RN, FNP).
  • Active DEA license or ability to obtain.
  • NPI number and furnishing license.
  • Ability to travel 20-30 miles from home-base to visit patients. 
  • An outstanding observation and analytical skill to diagnose and treat the patient effectively.
  • Excellent verbal, written, and interpersonal skills.
  • Fully bilingual in both English and Spanish is a plus.
  • Compassionate and skilled Nurse Practitioner.

Job Summary:

  • Provide clinical care and consultation.
  • Prescribe necessary treatment within the scope of practice and assess the patient's clinical problems and healthcare needs.
  • Prescribe medications in compliance with established medical protocols and in compliance with government agencies and guidelines.
  • Educate patients on preventive care and health safety issues.
  • Evaluate and treat patients in the skilled nursing facility setting or post-discharge at the patient's residential facility or private residence.
  • Perform annual wellness exams for senior Medicare patients at a residential facility or clinic setting.
  • Cover for other primary care physicians when necessary.
  • Evaluate and treat patients independently.
  • Obtain the patient's medical history and documentation in EMR.
  • Create a progress note for each encounter with each patient you evaluated. A progress note will include Subjective, Objective, Assessment, and Plan sections, and must be completed after each examination.
  • Promote a friendly, warm, and exceptional customer experience.
  • Secures patient information and maintains patient confidence by completing and safeguarding medical records; keeping patient information confidential by abiding by all HIPAA rules and regulations.
  • Prescribe pharmaceuticals, medications, and treatment regimens as appropriate to assessed medical conditions.
  • Any additional duties as assigned by the Medical Director, MDs, and Director of Operations.

Role as a Nurse Practitioner:
Nurse Practitioners (NPs) will step into the role of 'provider,' assessing patients referred by home health agencies to determine if they qualify as "homebound". Once patients are certified, they go back to the home health agency, which takes care of the actual home health care.

When visiting patients at home, NPs will conduct assessments including visual evaluations, necessary tests/labs, and physical exams. They’ll also review current medications to better understand the patient's conditions, like disability, confusion, or mental illness. All diagnoses will be documented in Office Ally. This role is similar to working in a clinic, but it's done at the patient's home.

NPs will start home health referrals as primary care providers, identifying patients who need extra care beyond their regular primary care. They’ll become skilled in diagnosis codes (our Chief NP and the team will train) to properly qualify patients for home health services, ensuring personalized attention.

NPs don't represent home health agencies directly, but rather are referred patients based on communication with prior physicians and home health agencies, with our NPs checking their eligibility. Our company bills Medicare directly, so each provider needs a unique PTAN (we will assist in signing you up as a Medicare provider). After assessment, NPs refer patients back to the home health agency.

NPs can also refer patients to hospice when needed or transition them from home health if necessary. NPs educate patients on prescriptions, ensure they’re used correctly, and can order labs and medications, including narcotics if they're DEA-licensed.

The goal is for NPs to provide personalized patient care, addressing the need for comprehensive attention rather than brief consultations with primary care providers. We believe that NPs, especially those with backgrounds in home health, patient care, or extensive nursing experience, have the insight and ability to meet patients' needs for enhanced care beyond what they have previously experienced.
